Consular Card Registration 

DEMANDE DE PASSEPORT ORDINAIRE

 

 Requirements 

  • One (1) request addressed to His Excellency the Ambassador
  • One (1) request (standard form) (click to access the form)
  • Three (3) recent passport photos taken face down on white background
  • One (1) photocopy of the old passport
  • One (1) extract or supplementary judgment of birth certificate
  • One (1) certificate of nationality or a legalized photocopy
  • One (1) photocopy of the Burkinabe identity card (CIB) or the consular card
  • One (1) certification of the profession (precise information)
  • One (1) judicial record (optional)

Payment

  • A money order for $100 if registered
  • A money order for $200 if unregistered
  • Made payable to the Embassy

DEMANDE DE PASSEPORT ORDINAIRE DE LES ENFANTS

 

Requirements

  •  One (1) request addressed to His Excellency the Ambassador
  •  One (1) request (standard form) (click to access the form)
  •  Three (3) photos of recent passports taken face to face on white background
  •  One (1) residence permit
  •  One (1) extract or supplementary judgment of birth certificate
  •  A certificate of nationality from Burkina Faso
  •  A parental authorization (only from the father or the mother)
  •  A legalized copy of the passport of the person giving the parental authorization
  •  A legalized copy of the Burkinabe Identity Card or the consular card of the person giving the parental authorization
  •  A school certificate if the child is enrolled in school
  •  Specify on request the size of the child

Payment

  • A money order for $100 if registered
  • A money order for $200 if unregistered
  • Made payable to the Embassy

REQUEST FOR RENEWAL OF REGULAR PASSPORT 

 

Requirements

  • One (1) request (standard form)
  • Three (3) recent passport photos taken face down on white background
  • One (1) copy of the passport
  • One (1) copy of the birth certificate or the supplementary birth certificate
  • One (1) copy of the Burkina Faso nationality certificate
  • One (1) photocopy of the Burkinabe identity card (CIB) or the Consular Card

Payment

  • A money order for $100 if registered
  • A money order for $200 if unregistered
  • Made payable to the Embassy

DEMANDE DE PASSEPORT DIPLOMATIQUE

 

Requirements

  •  One (1) request addressed to His Excellency the Ambassador
  •  One (1) request (standard form) (click to access the form)
  •  One (1) birth certificate or a birth certificate or a legalized copy
  •  One (1) legalized photocopy of the valid Burkinabè identity card (CIB)
  •  Two (2) recent passport photos taken face down on white background
  •  A document proving the applicant’s current qualifications and qualifications, which will be marked on the passport

Payment

  • A money order for $150 if registered
  • A money order for $300 if unregistered
  • Made payable to the Embassy
