The Marriage Record
Marriage Record Requirements
- A marriage application accompanied by a money order for $20
- Certificate of publication or non-opposition accompanied by a money order for $20
- An extract of the birth certificate of each of the future spouses or substitute judgment in lieu thereof
- A certificate of residence for each of the future spouses
- A certificate of premarital visit issued by a physician, if applicable
- A certificate of non-pregnancy, if applicable
- An administrative authorization from the superior, if applicable
- The consent of the father and mother, or of the person exercising parental authority
- The choice of the matrimonial regime adopted by the future spouses (wedding contract)
- As the case may be, the declaration of option of polygamy prior to the marriage
